SIGNATURE DUE BY TUESDAY JANUARY 7, 2020
1. In consideration of an offer to register or otherwise participate in Houston Girls Cotillion ("HGC"), or an event sponsored by HGC, each Member (as defined below) and her escort, as well as each of their parents and/or legal guardians ("Parent(s)") consent and agree to the terms of this Rules, Consent, and Waiver of Liability Agreement ("Agreement"). This full waiver, release, consent and agreement shall be effectuated by signing and returning this Agreement, after full review of its terms and granting full consent to such agreements stated herein. Each minor Member and her escort must consent and agree to all rules by signing the third page of this form, and each Parent (for both Member and escort) must also consent to all rules and release of liability by signing the third page of this form. Minors may not consent on behalf of their Parents.
2. Membership in HGC is by invitation only and is limited to the first 300 students who pay dues in full and complete on-line registration ("Member"). Registration to the event is not complete until this Agreement, signed by each Member, Parent, escort, and escort's Parent is received by the Houston Girls Cotillion, Class of 2021 Executive Board. All Agreements must be on file at the event.
3. Registration and payment information is provided on the Houston Girls Cotillion website: houstongirlscotillion.com / Class of 2021 (tab.) Complete, sign and return this Rules, Consent, and Waiver of Liability Agreement form by January 7, 2020 to HGC c/o Suzanne Stiles, 11757 Taylorcrest Rd, Houston, TX 77024, or email suzanne.stiles@yahoo.com. All Members are required to return a fully executed Agreement. Membership is limited to girls attending Duchesne Academy, Houston Christian High School, Lamar High School, Memorial High School, St. Agnes Academy and Second Baptist School. Members may only join the HGC class applicable to the member's expected graduation year,- 2021. Membership is not transferable unless approved by the Cotillion Board. Membership is not refundable under any circumstance.
4. Attendance at the event is limited to Members and their escorts. Attire is a long, floor-length formal gown or long pant jumpsuit for Members and a full tuxedo for gentlemen escorts. Texas tuxedos, boots or hats of any kind are not allowed. Inappropriate attire will be at the discretion of the Board. Members and escorts with inappropriate attire will not be admitted to the event and will not be eligible for any refund of monies.
5. No alcohol, tobacco, JUUL or drugs are allowed at the event. Members and their escorts, and their personal belongings, are subject to search at any time. Breathalyzer testing may be required at the discretion of HGC or security hired by HGC for the event. Any Member or escort that engages in unlawful acts or inappropriate behavior will be asked to leave immediately, and the Member's membership will be terminated and will not be eligible for any refund of monies.
6. Any Member or escort exhibiting unacceptable behavior (under the influence of alcohol, drugs, smoking, JUUL, fighting, lewd dancing, or foul language) or in the possession of alcohol, drugs or tobacco will not be allowed into the event and will be asked to leave immediately. HGC will attempt to contact a parent or the emergency contact on file for the Member or escort, but if HGC is unable to contact a parent or the listed emergency contact, the matter will be handed over to the police. If asked to leave, the Member's membership will be terminated and will not be eligible for any refund of monies.
7. Parents for each Member and escort accept full responsibility for their minor's observance of all rules and regulations, and are responsible for any damage caused directly or indirectly by their minor to the venue, any service provider, and/or any attendee or chaperone at the event.
8. Parking & Transportation: Parking is not available for Members and/or escorts, unless arriving with a Parent chaperone. All vehicles are subject to inspection prior to the event. Members and their escorts are prohibited from driving to the event. Members and their escorts must arrive and leave by driver. Driver can be a Parent, professional car service or Uber.
9. Members and their escorts may not arrive to the event after 8:00 p.m. and may not leave the event before 10:30 p.m. Members and their escorts are required to register with the check-in table upon arrival and must arrive by 8:00 p.m. DOORS WILL BE LOCKED PROMPTLY AT 8:00 p.m. Photo identification may be required for entrance.
10. Prior approval from the HGC Board must be obtained for any late arrivals or early departures and the Member and escort must check-in and/or check-out with an HGC representative.
11. Members and escorts will not be admitted to the event unless HGC has received a complete registration for the Member and escort, and a signed Rules, Consent, and Waiver of Liability Agreement acknowledging that the Member, her escort and each Parent agree to the terms of this Agreement.
12. Backpacks, oversized purses, and overnight bags are not allowed. Coats, shoes and purses may be checked but HGC is not responsible for any damage or loss relating thereto.
13. Failure to comply with the terms of this Agreement is grounds for dismissal from the event without any refund.
14. FULL RELEASE AND INDEMNITY. HGC is an organization of volunteers. In consideration of an offer to register or otherwise participate in an event sponsored by HGC (on behalf of themselves as Members, or as the escort of a Member or the Parent of a Member or escort); each Member, escort, and Parent agrees, for themselves and for and on behalf of their minors, WAIVE, RELEASE and FOREVER HOLD HGC, its members, Board and volunteers (collectively "HGC") HARMLESS from any and all damages, liabilities, causes of action, claims, and losses whatsoever arising as a result of negligence by HGC in connection with the organization or the event. Each Member, escort, and Parent further agree to INDEMNIFY HGC relating to any losses, damages, claims, or causes of action directly or indirectly relating to their, or their minor's, participation in HGC, including, but not limited to, the event, and any activities prior to and after the event. Participation in the event is voluntary and the Member, escort, and Parents assume all risk associated therewith. HGC is not responsible for the safety or conduct of any Member or escort.
15. The Board of the Houston Girls Cotillion - Class 2021 reserves full discretion in admitting and dismissing Members and escorts, with no expectation of any refund of any kind.
16. Members may attend the event with or without a gentleman escort.
17. Member, escort and Parents each acknowledge that there will be photographs and video taken during the event, and each consent and agree to the use of their photos and video on the website of HGC or any photographer designated by HGC as the event photographer. HGC is not responsible for the content of the photos or video and any complaints related thereto should be directed to the photographer. This consent and release is without further compensation for the use of such photos and video.
If Member and Parent elects to opt-out of signing the Consent Agreement electronically, the Member and Parent may print the Consent Agreement Form from the www.houstongirlscotillion.com website, manually sign and mail to HGC, c/o Suzanne Stiles, 11757 Taylorcrest Rd, Houston, TX 77024, or email suzanne.stiles@yahoo.com.
All signatures are due on Tuesday, January 7, 2020.
